You might have encountered some issues with the web applications still displaying the URL bar even after setting the web application to full screen: 
This is caused by site redirection. For example, you might have set the URL to youtube.com, however, if the site detects that you are using a mobile device, it will redirect to m.youtube.com, that is why the bar becomes visible. Some sites do not have a redirection link, that's why sometimes, it works.
This is the same with Google Forms since users have the option to shorten the URLs.
If you want to keep the URL bar hidden, you can change the URLs of your web apps to avoid redirection. To force the update of the app, you can also remove the app from the profile, do the edit, and then re-add the app and save the profile.
